(Over) On back of survey sheet (Verso): Was a member of the regular naval forces from Sept. 12, 1909 until after the late war, being discharged April 10th, 1919. During the war, I served on board the Destroyer Benham in European waters, which was one of the very first of American forces to arrive on the other side. In the period of time in which I served on board the Benham, we engaged twelve submarines, was shipwrecked on the night of August 20, 1919 sixty miles from the south Irish coast, lately rescued by the British sloop of war Zinia. The Benham was salvaged by American forces, repaired and renamed by its original crew. On the morning of Dec. 17, 1917, we picked up 29 survivors of a torpedoed British fr[e]ighter, and three hours later was nearly shipwrecked a second time in [?}...[the remainder of the line is glued to he scrapbook page].
